Refusing treatment: writing your own refusal

If you would like to refuse treatment in a situation that isn’t covered by the scenarios in this form, you can write it here. For example, some people want to refuse CPR in every situation.

The scenarios you have seen so far all contain a refusal of ‘life-sustaining treatment’ but you might want to refuse a treatment that isn’t life-sustaining. You might want to do this, for example, if you are allergic to certain medications. Also, you might not want to refuse all life-sustaining treatment and instead just refuse specific ones.

You must write the treatments that you want to refuse and the situations in which you want to refuse them. To be legally binding, it is not enough to write a general wish to not be treated.
